Partner Access Pathway

The Partner Access Pathway is a formal, funded partnership model that allows agencies and organizations to refer a larger number of families to the Tiny Essentials Relief Hub while supporting the staffing, inventory, and infrastructure needed to sustain prevention through access.

Through this partnership, agencies help extend the reach of the Relief Hub, ensuring that families across Philadelphia have consistent access to essentials and support. In return, Linked Family Services provides structured coordination, data reporting, and partnership recognition.

How It Works

  • Designed for agencies, hospitals, and programs with available funding to support collaboration.

  • Includes an executed Memorandum of Understanding (MOU) and/or funding agreement.

  • Offers expanded referral access beyond the standard 10-family monthly limit.

  • Families are scheduled directly through the LFS referral process.

Becoming a Partner Access Pathway Agency

To establish a funded partnership with Linked Family Services:

  2. Schedule a brief partnership discussion to outline your agency’s needs and available resources.

  3. Execute a funding agreement and MOU to finalize your participation.

If your agency does not currently have funding available to support a funded partnership, you may consider joining our Community Partner Connection →, which allows up to 10 family referrals per month under a standard MOU.
