At Linked Family Services, we believe that tiny essentials are a big deal. The Tiny Essentials Relief Hub is a prevention-focused resource operated by Linked Family Services.
The Hub provides diapers, wipes, formula, period/menstrual products, child & adult hygiene items for families with children under age 5 — but it’s more than just supplies. It’s a welcoming space where families connect with dignity, care, and support.

how it works
At Linked Family Services, we believe that access to essential baby and family items is one of the most effective ways to reduce caregiver stress, strengthen family stability, and prevent crises before they occur.
Our Essential Goods Access program operates through the Tiny Essentials Relief Hub (TERH)—a prevention-focused care coordination program that pairs concrete supports with relationship-based services designed to help families thrive.
Every participating family is connected with a dedicated Care Coordinator who serves as their primary point of contact throughout the program. Care Coordinators provide ongoing support, resource navigation, referrals, and connections to prevention-focused services that strengthen protective factors and promote long-term family well-being.

How to Access Services
Services are provided through the Tiny Essentials Relief Hub using a structured, appointment-based model.
Step 1
Complete the online Referral & Interest Form.
Step 2
A Care Coordinator will review your information and contact you to schedule an in-person intake appointment.
Step 3
Attend your intake appointment to discuss your family's needs, complete enrollment, and determine eligibility for ongoing services.
Who Is Eligible?
Families must:

Be pregnant and/or parenting a child ages 0–8.
-
Reside in Philadelphia County.
There is no insurance requirement, and all services are provided free of charge.

Our Prevention Approach
The Tiny Essentials Relief Hub is more than a resource distribution program.
Through prevention-focused care coordination, we partner with families to reduce barriers, strengthen protective factors, address Health-Related Social Needs (HRSNs), and connect caregivers to resources before challenges become crises.
Every family receives individualized support based on their strengths, priorities, and goals, while maintaining access to essential goods, educational programming, wellness opportunities, and community partnerships.
When caregivers have consistent access to essential items, trusted relationships, and coordinated support, they are better equipped to care for their children, strengthen family stability, and build healthy futures.
