Families enrolled in one of the following LFS programs may become eligible after 3 months of consistent engagement:

The PILLAR Program (caregivers of children ages 4 months to 5 years)

MOMcares Prenatal Outreach Program (pregnant and postpartum parents)

Completion of 3 months of program engagement (visits, check-ins, milestone tasks)

Attendance at an in-person Infant Safe Sleep Training

Home delivery provided (setup not included)

This is an internal program benefit and is not available by public request or referral.
At Linked Family Services, we believe every baby deserves a safe place to sleep. Through our SleepSafe Response Service, we provide Pack ’n Plays or bassinets to families who meet specific eligibility criteria. This is a milestone-based incentive, not a public giveaway.
We offer this service in two ways:

We also provide emergency Pack ’n Play or bassinet delivery to families referred by agency partners. This service is available to organizations with a signed MOU and includes:

Emergency Referrals from Partner Agencies + Reimbursement Policy
Social service agencies and community partners can refer eligible families to receive a Pack 'n Play or bassinet through the SleepSafe Response Program on an emergency basis to support family reunification or to prevent the removal of a child from the home due to unsafe sleeping arrangements.

The caregiver must complete the virtual Cribs for Kids® Infant Safe Sleep Training

A certificate of completion must be submitted to LFS before delivery is scheduled
-

All items are delivered directly to the family within 7 days of an approved crib request packet, including submission of the completed Infant Safe Sleep Certificate.

To help sustain this service, a reimbursement agreement is required.
-
Referring agencies must have a signed MOU on file with Linked Family Services.
-
Agencies agree to remit the cost per item to Linked Family Services within 30 days of invoice.

LFS will provide:

Delivery confirmation
-
Invoice with payment instructions
-
Optional follow-up documentation upon request
-
ONE Pack 'n Play or bassinet (home delivery only; no setup included)

Safe Sleep education for the family

To request an MOU, submit an agency referral
Agency Crib Request Packet
To qualify for delivery, the following items must be submitted in full:

Completed Agency Referral Form or Self-Referral Form.
-
All sections filled out, including family and child information, current sleep environment, and reason for referral.

Signed Consent Confirmation
-
Agency affirms that permission was obtained from the parent/guardian to submit the referral on their behalf.

Parent affirms that they are willingly submitting the referral and agree to the terms of participating in the MOMCares Prenatal Outreach program.

Infant Safe Sleep Certificate
-
Certificate of completion from the caregiver showing they passed the online Infant Safe Sleep Training (90% or higher).

May be submitted as a PDF, screenshot, or printed copy.

Signed MOU on File (for referring agency only)

Agencies must have a current Memorandum of Understanding with Linked Family Services before submitting referrals. If you not sure of the status of an MOU for your organization, please contact us via email at contact@linkedfamily.org.

Once all documents are received and reviewed, eligible families will receive a Pack ‘n Play or bassinet within 7 business days.
